Endomysial antibodies (EMA) are autoantibodies, primarily of the IgA class, that bind to endomysium—the thin connective tissue layer covering muscle fibers. They are highly specific for celiac disease, an autoimmune disorder triggered by gluten ingestion in genetically predisposed individuals. The test detects these antibodies in the blood using indirect immunofluorescence on tissue substrates.
Because of its high specificity, it is often used as a confirmatory test following positive serology for tissue transglutaminase antibodies. It is not a screening test due to higher cost and technical demands.
