ME/CFS is known for standard lab parameters often sitting within the normal range even though patients are severely affected. Abnormalities often only show up in more specific markers (NK cell function, hsCRP, HPA axis) or over time – not in a single value.
The reference range is based on population statistics. The "optimal" range is narrower and oriented towards clinical symptom freedom. Example: ferritin >15 µg/l counts as "normal", but for fatigue a value >50–100 µg/l is clinically desirable.
