Activated partial thromboplastin time (aPTT) measures how long citrated plasma takes to clot after specific reagents are added. It evaluates much of the intrinsic and common coagulation pathways and is used in settings such as unexplained bleeding and monitoring unfractionated heparin.
A prolonged aPTT can result from congenital or acquired clotting-factor deficiencies, heparin, lupus anticoagulant or other coagulation inhibitors, as well as conditions such as fibrinogen deficiency, liver disease or severe vitamin K deficiency. The direction of the abnormality does not identify the cause by itself. Depending on the clinical question, mixing studies and targeted factor or inhibitor testing may be used.
A shortened aPTT should not be treated as a direct test for factor V Leiden or inherited thrombophilia. Mayo Clinic Laboratories notes that shortening most often reflects increased factor VIII activity associated with acute or chronic illness or inflammation, or spurious effects from difficult venipuncture, specimen collection or processing. A normal or shortened aPTT also does not exclude a hemostatic disorder.
Reference intervals vary with reagent and laboratory. Medicines and sample quality can affect the result, so treatment changes should not be made from an isolated aPTT value.
