Troponin I (cTnI) is part of the troponin complex that regulates heart muscle contraction. It is found exclusively in cardiac myocytes, making it a highly specific indicator of heart injury. When heart cells are damaged—for example, during a myocardial infarction—troponin I leaks into the circulation, where it can be measured with high-sensitivity assays.
Measuring troponin I is a cornerstone in the rapid diagnosis of acute coronary syndromes, especially when a person presents with chest pain, shortness of breath, or other symptoms suggestive of a heart problem. Levels rise within hours of injury, peak around 12–24 hours, and can remain elevated for up to two weeks.
Because even minor damage causes a detectable rise, troponin I is used not only to diagnose heart attacks but also to assess heart muscle injury in conditions such as myocarditis, pulmonary embolism, sepsis, and heart failure, as well as after cardiac procedures.
