p-Cresyl sulfate, often abbreviated as pCS, is a small organic molecule that originates in the gut through bacterial fermentation of the amino acids tyrosine and phenylalanine. After absorption into the bloodstream, it undergoes sulfation in the liver before being excreted by the kidneys. In healthy individuals, renal clearance keeps pCS levels low; however, in chronic kidney disease (CKD), impaired kidney function leads to its accumulation.
Elevated pCS is considered a uremic toxin and has been linked to progression of kidney disease and increased cardiovascular complications. Testing for p-cresyl sulfate is primarily used in research settings and specialized nephrology clinics to evaluate toxin burden and related risks in patients with CKD.
