Active Vitamin B12, also known as holo-transcobalamin (Holo-TC), represents the portion of vitamin B12 that is bound to transcobalamin, the carrier protein that delivers B12 to cells throughout the body. Unlike a total B12 test, which measures all circulating B12 (including forms that may not be readily usable), the active B12 test specifically quantifies the biologically available fraction.
Measuring active B12 is particularly useful for early detection of functional vitamin B12 deficiency, since it may reveal a deficiency before total B12 levels fall below the reference range. This can be important for investigating symptoms such as fatigue, cognitive difficulties, or neurological issues even when standard B12 results are borderline or within the normal range.
