Anti-GBM (anti-glomerular basement membrane) antibodies are autoantibodies that attack the glomerular basement membrane, a key structure in the kidneys' filtering units, and occasionally the lung basement membrane. The presence of these antibodies is the hallmark of Goodpasture syndrome (also called anti-GBM disease), a rare but severe autoimmune condition. When anti-GBM antibodies bind to the basement membranes, they trigger inflammation and damage, leading to rapid kidney failure (glomerulonephritis) and, in many cases, lung hemorrhage. This disease can progress quickly, so early detection is critical.
Testing for anti-GBM antibodies is essential for diagnosing Goodpasture syndrome and distinguishing it from other causes of kidney or lung bleeding. The test is usually performed on blood using ELISA or indirect immunofluorescence, and results are often reported as positive or negative, or sometimes as a titer. A positive result, combined with typical symptoms and biopsy findings, confirms the diagnosis and typically warrants urgent medical intervention to suppress the immune response and remove the antibodies.
