Reticulocyte hemoglobin (Ret-Hb) is a measurement of the hemoglobin content within reticulocytes, the immature red blood cells recently released from the bone marrow. Because reticulocytes circulate for only 1–2 days before maturing, Ret-Hb reflects the iron status over the preceding few days, making it a sensitive indicator of iron available for red cell production. Unlike ferritin, which can rise during inflammation or illness, Ret-Hb is not influenced by acute-phase responses, so it remains accurate even when an infection is present.
Measuring Ret-Hb is particularly useful for the early detection of iron deficiency before anemia develops, as well as for monitoring the response to iron therapy. Low Ret-Hb values can signal functional iron deficiency, where the body has enough total iron stores but cannot effectively use them for hemoglobin synthesis. The test is often ordered alongside a complete blood count (CBC) and other iron studies to get a comprehensive picture of iron metabolism.
