Anti-Smith antibodies (anti-Sm) target Smith antigens, which are components of the spliceosome complex within cells. They are considered a highly specific serological marker for systemic lupus erythematosus (SLE) and are included in the classification criteria for this autoimmune disease. A quantitative anti-Sm test measures the concentration of these autoantibodies, aiding in diagnosis and differentiation from other connective tissue diseases. However, not all SLE patients test positive; anti-Sm is present in about 20–30% of cases, but a positive result strongly supports the diagnosis.
