M3 muscarinic acetylcholine receptor autoantibodies are immune proteins that mistakenly target the M3 subtype of muscarinic acetylcholine receptors. These receptors are located on the surface of smooth muscle cells and exocrine glands, including salivary and tear glands. Under normal conditions, acetylcholine binds to these receptors to stimulate glandular secretions.
When autoantibodies interfere with this process, it can lead to reduced saliva and tear production, contributing to the hallmark dryness of Sjögren’s syndrome. Measurement of M3-AChR autoantibodies in the blood may assist in the diagnosis of Sjögren’s syndrome, especially in cases where typical antibodies (such as anti-SSA/Ro) are negative. While not a first-line test, it can provide supportive evidence of autoimmune-mediated glandular dysfunction.
Testing is performed using enzyme-linked immunosorbent assay (ELISA) or similar immunoassay methods, and results are typically reported qualitatively as positive or negative, or sometimes semi-quantitatively as a titer.
