Rheumatoid factor (RF-IgM) is an immunoglobulin M (IgM) antibody that binds to the Fc portion of other antibodies (IgG). In healthy individuals, RF is typically absent or present at very low levels. However, in certain autoimmune diseases, most notably rheumatoid arthritis, the immune system produces higher amounts of RF-IgM, which can form immune complexes and contribute to inflammation, particularly in the joints.
Measuring RF-IgM in the blood is a common part of the diagnostic workup for rheumatoid arthritis, though it is not specific to the disease and can be elevated in other conditions as well. Elevated RF-IgM levels are seen in approximately 70–80% of people with rheumatoid arthritis, but they can also occur in other autoimmune disorders (such as Sjögren’s
syndrome or lupus), chronic infections (like hepatitis C or tuberculosis), and even in some healthy older adults. Therefore, the test result must be interpreted alongside clinical symptoms, physical examination, and other laboratory findings, such as anti-cyclic citrullinated peptide (anti-CCP) antibodies, to help establish an accurate diagnosis.
