Free triiodothyronine (fT3) is the unbound fraction of the thyroid hormone T3 circulating in the blood. T3 is largely produced by conversion from thyroxine (T4) in organs like the liver and kidneys and is the most potent thyroid hormone, driving metabolic rate, heart function, and development. Because only the free fraction is active, measuring fT3 provides a direct assessment of thyroid status.
fT3 is especially useful in diagnosing and monitoring hyperthyroidism. It can be elevated in Graves’ disease or toxic nodules, while it may be low in non-thyroidal illness or hypothyroidism. Alongside TSH and free T4, fT3 helps distinguish different thyroid disorders and guide treatment.
