Neurofilament light chain (NfL) is a structural protein found primarily in the long fibers (axons) of nerve cells. When axons are injured or degenerate, NfL is released into the surrounding fluid and eventually reaches the bloodstream. Measuring NfL in serum provides a minimally invasive way to assess neuroaxonal damage.
The serum NfL test is used as a biomarker for various neurological disorders. It can help in the evaluation of conditions like multiple sclerosis, Alzheimer’s disease, amyotrophic lateral sclerosis (ALS), and traumatic brain injury. Because NfL levels tend to reflect the extent of ongoing nerve cell damage, serial measurements may be used to monitor disease progression or response to treatment. However, elevated NfL is not specific to one disease and must be interpreted within the full clinical context.
