Phosphorylated tau at threonine 181, or p-Tau181, is a specific form of the tau protein that has been chemically altered by the addition of a phosphate group. Tau proteins normally stabilize microtubules in neurons, but in Alzheimer’s disease and other tauopathies, hyperphosphorylation leads to the formation of neurofibrillary tangles, a hallmark of neurodegeneration.
p-Tau181 can be measured in cerebrospinal fluid (CSF) obtained by lumbar puncture, and increasingly in blood plasma, as a biomarker to support the diagnosis of Alzheimer’s disease. Elevated p-Tau181 levels are associated with amyloid beta pathology and can help differentiate Alzheimer’s from other causes of dementia. This test is typically used by specialists in memory clinics and neurology.
