Neurofilament light chain (NfL) is a protein that forms part of the internal skeleton of nerve cells. When central nervous system axons are damaged, NfL is released into the cerebrospinal fluid (CSF). CSF NfL measurement is a direct, sensitive indicator of neuroaxonal injury within the brain and spinal cord.
The CSF NfL test is used in the investigation of neurodegenerative and inflammatory disorders, such as Alzheimer’s disease, amyotrophic lateral sclerosis (ALS), multiple sclerosis, and frontotemporal dementia. It can also be elevated after acute injuries like spinal cord trauma or stroke. Because CSF offers a “window” into the central nervous system, NfL levels in CSF tend to be
higher than in blood and may provide earlier or more pronounced evidence of nerve cell damage. Interpretation must account for the clinical picture and laboratory‑specific reference ranges.
