Parietal cell antibodies, also known as anti-parietal cell antibodies (APCA), are autoantibodies directed against the gastric parietal cells. These specialized cells in the stomach lining secrete gastric acid and intrinsic factor, which is essential for vitamin B12 absorption. The presence of APCA in the blood indicates an autoimmune response against these cells, leading to their destruction.
This is a hallmark of autoimmune atrophic gastritis, a condition that can result in vitamin B12 deficiency and pernicious anemia. Testing for APCA is often part of the evaluation for unexplained anemia, gastrointestinal symptoms, or vitamin B12 malabsorption. Results are typically reported in arbitrary units per milliliter (AU/mL), with a reference range that is lab-dependent; often a value below a certain cutoff (e.g., <1:20 titer) is considered negative.
